Transport of Ashes or Human Remains
Requirements for the transport of ashes or human remains:
Required documentation for the transport of ashes:
- Apostilled death certificate issued by the Australian Department of Foreign Affairs and Trade (DFAT) or the New Zealand Ministry of Internal Affairs, as applicable
- Copy of Uruguayan passport or ID card
- Cremation certificate
- Signed letter from the authorised person formally requesting the procedure (form available at the bottom of the page)
Required documentation for the transport of human remains:
- Apostilled death certificate issued by the Australian Department of Foreign Affairs and Trade (DFAT) or the New Zealand Ministry of Internal Affairs, as applicable
- Copy of the deceased person’s identity document (preferably a Uruguayan document)
- Signed letter from the authorised person formally requesting the procedure (form available at the bottom of the page)
- Power of attorney or written authorisation to carry out the procedure
- Embalming certificate and certificate confirming hermetic sealing of the coffin, duly endorsed by a medical professional and apostilled
- Authorisation from the relevant health authority for the transport of the remains
