  • Apostilled death certificate issued by the Australian Department of Foreign Affairs and Trade (DFAT) or the New Zealand Ministry of Internal Affairs, as applicable
  • Copy of the deceased person’s identity document (preferably a Uruguayan document)
  • Signed letter from the authorised person formally requesting the procedure (form available at the bottom of the page)
  • Power of attorney or written authorisation to carry out the procedure
  • Embalming certificate and certificate confirming hermetic sealing of the coffin, duly endorsed by a medical professional and apostilled
  • Authorisation from the relevant health authority for the transport of the remains
