Registration of children born in Israel

Any child or grandchild of a Uruguayan (born in Uruguay) must register their birth in the Uruguayan Civil Registry in order to apply for their first passport or to renew it, as well as to renew their Uruguayan identity card.

Requirements for children of Uruguayans (Law 16.021)

•    Israeli birth certificate printed in Hebrew and English and apostilled by the Ministry of Foreign Affairs (Jerusalem). 
•    Valid Uruguayan birth certificate of the mother/father born in Uruguay (issued less than 30 days ago).
•    Two witnesses who will appear on the day of registration with a valid passport. The parents, siblings, children, or spouse of the applicant cannot be witnesses.

Notes: If the applicant and/or their Uruguayan parent has changed their name and/or surname, and the names on the Israeli certificates do not exactly match the Uruguayan documents, a name change certificate apostilled and translated by the Consul in Ashdod (juliol@conuruguayash.co.il) or by the Consul in Zichron Yaakov (04-6398460) must be presented. The name change is requested at the Israeli Ministry of the Interior and apostilled at the Israeli Ministry of Foreign Affairs.  

To register minors, both parents must be present, each with a valid passport and the required documentation mentioned above, as well as the two witnesses.

Requirements for grandchildren of Uruguayans (Law 19.362)

•    Israeli birth certificate printed in Hebrew and English and apostilled by the Ministry of Foreign Affairs (Jerusalem). 
•    Uruguayan birth certificate of the father or mother if available, or Israeli birth certificate printed in Hebrew and English and apostilled by the Ministry of Foreign Affairs (Jerusalem). 
•    Valid Uruguayan birth certificate of the mother/father born in Uruguay (issued less than 30 days ago).

Registration of deceased persons in Israel

Requirements

•    Israeli death certificate apostilled by the Ministry of Foreign Affairs (Jerusalem). 
•    Uruguayan Identity card of the deceased or their Uruguayan passport.
•    Translation of the document carried out either by the Consul in Ashdod (juliol@conuruguayash.co.il) or by the Consul in Zichron Yaakov (04-6398460).
•    Two witnesses who will appear on the day of registration with a valid Uruguayan identity card or a valid passport.
