Minor’s Authorization to Renew or Issue a Uruguayan Passport

For interested parties who wish to authorize a minor to issue or renew an ordinary Uruguayan passport.

Requirements

  1. Valid identity document, in good condition, of both the mother and the father.
  2. Valid birth certificate or birth record of the minor, issued less than 30 days prior.
  3. Copy of the minor’s valid identity document.
  4. Application form, available at the bottom of the page.

* If the minor has not been registered with the General Directorate of Civil Registry of Uruguay, the registration may be carried out at the Consulate in Ottawa or Gatineau provided that the birth occurred within the jurisdiction of that Consular Office.

When one of the parents is deceased:

  • Appearance of the surviving parent to sign the minor’s authorization, with their valid identity document in good condition.
  • Minor’s identity card in good condition.
  • Birth certificate issued less than one year prior, to prove the parent-child relationship.
  • Death certificate.
